Miami is one of the most diverse and high-intensity cities in the country — multilingual households, demanding schools, busy professional parents, and the constant sensory load of urban South Florida life. For neurodivergent kids growing up in Coral Gables, Coconut Grove, Pinecrest, or Brickell — and for the many adults here figuring out late-diagnosis ADHD, autism, or AuDHD — the right OT support has to meet a more complex daily life than the standard pediatric-clinic model assumes.
Ocean Tide Therapy is led by Stefanie Irmen, OTR/L, a licensed occupational therapist with a decade of experience across school-based, outpatient, and developmental settings. Miami is one of our home markets, and we built the practice specifically to fit the diversity of families that the city actually contains.
How sessions work: in-home OT covers the full Miami-Dade area — Coral Gables, Coconut Grove, Pinecrest, Palmetto Bay, South Miami, Brickell, Edgewater, Wynwood-adjacent, Key Biscayne, and the surrounding neighborhoods. We come to your home, condo, or apartment — meeting your family in the actual environment your kid lives in. For families who prefer telehealth (or whose Miami traffic and schedule make in-home tricky), HIPAA-compliant video sessions are available throughout Florida.
A note on language: we currently serve clients in English only. If your family needs Spanish-language OT, we want to be honest about that limitation up front rather than overpromise. Our approach is explicitly affirming, identity-first, culturally aware, and grounded in the actual research on what supports — rather than harms — neurodivergent nervous systems.

Adult OT details →What Miami families typically come in for
The high-pressure schools piece. Miami's top schools — Gulliver Prep, Ransom Everglades, Carrollton, MAST, Coral Reef, Coral Gables Senior — are academically intense and competitively selected. Many neurodivergent kids hold it together at school all day and completely fall apart at home. We work with the kid, the family, and (with your consent) the school team to close the gap between performance and what your kid's nervous system can actually sustain.
The multilingual household piece. Many Miami families speak Spanish, Portuguese, French, Creole, or Russian at home, with English at school. Most pediatric therapy ignores how that affects sensory load, language processing, and the cognitive cost of switching contexts. We don't. We work with multilingual families regularly and account for the real load that switching codes adds to a neurodivergent nervous system.
The late-diagnosed adult piece. Miami's professional adult community has a growing population of late-diagnosed Autistic, ADHD, and AuDHD adults — often surfaced after a child's diagnosis or a burnout episode. The high-rise sensory piece. If you live in Brickell, Edgewater, or anywhere along the bay in a vertical building — your nervous system is dealing with elevator transitions, hallway acoustics, hurricane impact glass, AC noise, neighbors above and below, and the constant low-grade urban sensory load that ground-floor suburban living doesn't have. We design sensory strategies for the actual building you live in.
Common questions from Miami families
Do you actually come to homes in Miami-Dade, or is this telehealth-only?
Both. In-home OT covers the full Miami-Dade area — Coral Gables, Coconut Grove, Pinecrest, Brickell, Edgewater, and surrounding neighborhoods. For families whose schedule (or Miami traffic) makes in-home tough, we also offer secure HIPAA-compliant telehealth throughout Florida.
Do you have Spanish-speaking OTs?
Not currently. Sessions are in English. We're naming this honestly rather than overpromising — we'd rather refer you to a Spanish-speaking provider than provide care in a language we can't fully support. If you'd like, we can help you find Spanish-speaking affirming OTs in the Miami area.
How is this different from other pediatric OT clinics in Miami?
Two real differences. First, the model: we come to you, in your space, on your child's timeline — not a 45-minute clinic slot. Second, the philosophy: many Miami pediatric clinics still use compliance-based approaches (sticker charts, planned ignoring, "first/then" as control, exposure-based feeding). We don't. Do you take Aetna or BCBS?
We are in-network with Aetna in Florida now. BlueCross BlueShield of Florida is coming soon. We also accept private pay, HSA/FSA, and provide superbills for out-of-network reimbursement (most PPO plans reimburse a meaningful portion).
My child is currently in ABA. Can we do OT alongside it?
Yes — many families add affirming OT alongside ABA, and over time some families transition fully to OT-based approaches. We're not here to tell you what to do with other services. If you have questions about how the two approaches differ philosophically, we'll talk through that on the consultation.
Will you collaborate with my child's school in Miami?
With your written consent, yes. We work with Miami-Dade County Public Schools, charters, magnets, and major independents. We're familiar with the IEP and 504 process in Miami-Dade and can advocate inside the system or help you navigate around it where needed.
