What Is Neuroaffirmative Occupational Therapy?

For decades, occupational therapy operated from a single premise: identify what the person can't do, then fix it. That model works well for many things — recovering from a stroke, rebuilding strength after surgery. But when it comes to neurodivergent individuals — people who are autistic, ADHD, dyslexic, or otherwise differently wired — the "fix it" model has caused real harm.

Neuroaffirmative occupational therapy starts from a completely different place: your brain is not broken.

What does "neuroaffirmative" actually mean?

Neuroaffirmative care is grounded in the neurodiversity paradigm — the idea that neurological differences like autism and ADHD are natural variations of the human brain, not disorders to be eliminated. Neuroaffirmative therapists work with how your brain functions, rather than trying to make it behave like a neurotypical brain.

In practice, this looks like:

How is this different from traditional OT?

Traditional OT might see an autistic child who hates eye contact and work to increase eye contact. Neuroaffirmative OT asks: why are we targeting this? Is it serving the child, or is it serving the adults around the child? If it doesn't improve the child's quality of life and autonomy, we don't pursue it.

The difference shows up in goals, too. Traditional: "reduce stimming behavior." Neuroaffirmative: "help the child identify when stimming is meeting a need and advocate for environments where they can do so freely."

Who benefits from neuroaffirmative OT?

Anyone who is neurodivergent can benefit — autistic individuals, those with ADHD, sensory processing differences, dyspraxia, twice-exceptional learners, and more. It is also helpful for people who have had negative experiences with traditional therapy and need a different approach to feel safe enough to engage.
